The cost checklist & worksheet
Make the whole bill visible.
Ask about the starting bill, charges that repeat, and the price once an offer has ended. Put missing fees on your question list.
Questions that expose a partial price.
Ask for the repeat bill, not just the opening offer.
In the check dated September 26, 2026, most online weight-care services billed membership and medicine as separate charges.
What does the membership include? Are later visits, lab tests and delivery extra? What do I pay if no prescription is issued?
Keep a medicine price separate from a care price.
On September 26, 2026, the maker’s monthly semaglutide cash prices ran from $149 to $499 across products and strengths.
The lowest injection offers were for new cash-pay patients and covered only the first two monthly fills.
Those introductory terms ran through December 31, 2026, as recorded on September 26, 2026.
A medicine-only price does not answer what the online care will cost. Ask for the amount after any offer and the terms for leaving.
Ask the insurer about the exact product and use.
As recorded on September 26, 2026, Medicare drug coverage can depend on a medicine’s prescribed use, the plan’s list of medicines and prior approval.
Prior approval means the insurer reviews the request before agreeing to pay. Ask what you would owe if approval is refused.
Could the Medicare program apply to you?
People meeting the Medicare GLP-1 Bridge rules could get certain weight medicines with a monthly copay of $50, as recorded on September 26, 2026.
A copay is the part of a bill that the patient pays.
GLP-1 is a hormone from the gut: this body signal plays a part in blood sugar and appetite.
Medicines such as semaglutide copy its action.
Semaglutide (Wegovy, Ozempic) has different approved products for different uses.
For semaglutide, the program included Wegovy injections and tablets, not Ozempic for weight loss.
Medicare’s agency gave the program a start date of July 1, 2026, and an end date of December 31, 2027.
The program needs both a prescription and an approved request before it can pay.
These are the main rules recorded on the date above. Your plan and the person prescribing the medicine need to check all the program’s rules for your case.
- You must be at least 18.
- To qualify, your plan must be either a separate drug plan under Medicare Part D or a Medicare Advantage coordinated care plan that includes drug coverage. Ask your plan to confirm that its exact type is included.
- The program uses your weight measure, called body mass index or BMI, and health conditions when GLP-1 treatment began. Ask the person treating you to check those records with you.
- No extra health condition is needed to meet the weight rule when BMI is 35 or higher.
A BMI of 30 or higher must come with at least one of these conditions:
- Heart failure with the medical-record name “preserved ejection fraction”.
- Blood pressure that stays above 140 for the upper number or above 90 for the lower number despite two blood-pressure medicines.
- Kidney disease in stage 3a or a later stage.
A BMI of 27 or higher must come with at least one of these conditions:
- Pre-diabetes, meaning blood sugar that is high but not yet type 2 diabetes.
- A past heart attack or stroke.
- Peripheral artery disease that causes symptoms. Ask whether that exact diagnosis appears in your medical record.
- Type 2 diabetes uses the regular Part D coverage route instead of this program.
- Certain sleep and liver illnesses, and prescriptions to lower heart-attack or stroke risk, also use the regular Part D route. Ask which route fits your diagnosis.
- If Part D pays for your GLP-1 medicine in 2026, you cannot use the Bridge during 2026.
- Treatment must include ongoing help with food choices and physical activity.
The $50 is still money you pay, but the program does not add it to the spending counted toward your Part D plan’s yearly limit.
Reaching that limit for other covered medicines does not remove this monthly payment.
Check the Medicare program’s termsPut your own quote on paper.
Use amounts a service has quoted to you. Enter a zero only when a fee is confirmed as free or included.
Amounts are in US dollars. Leave an unknown fee blank. For repeat costs, enter the amount for a month.

The first-month sum adds your start costs to one month of repeat costs. Blank fees are still missing from the sum. Entries stay on this page and clear when it reloads.

These were the maker’s listed medicine prices on the date shown. Ask for a current quote and check when any offer ends. The medicine price leaves online visits and other fees unanswered. Medicare’s page explains the temporary program.
